Reporting an Academic Integrity Incident

There are several main categories and types of academic dishonesty. 

  • Once you’ve observed and have evidence of an academic integrity violation, inform the student and meet with them to describe the situation and present your evidence that a violation has occurred. Give the student a chance to respond to the situation.
  • Submit the Incident Report. You decide what sanction(s), including to not assign a sanction, you will impose based on what you have written into your syllabus and the academic integrity policy. 
  • An email will be sent to the student by the Office of Academic Affairs explaining the incident as you described it and any sanction you intend to impose.
  • The student may accept your sanction or appeal it to the department chair or program coordinator. If the student accepts your sanction, you may impose it. Sanctions such as failing the course can be recommended but cannot be assigned until the student's appeal rights are exhausted. If they choose to appeal, students have the right to continue attending class and completing assignments until the appeal process has run its course.
  • Once a student appeals, he/she/they take the responsibility to contact and meet with the department chair or program coordinator. If you are the department chair or program coordinator, the student must be notifed to make their appeal to your direct supervisor, or if appropriate, another department chair or program coordinator. An email will be sent to you notifying you of a student's appeal to the department head.
  • The Office of Academic Affairs will provide the materials to the department chair, program coordinator or immediate supervisor and the Academic Integrity Council panel for review.
  • The department chair, program coordinator or immediate supervisor completes the Department Head Decision form, and you and the student will receive a letter via email explaining his/her/their decision. 
  • If the department head, program coordinator, or direct supervisor's decision does not resolve the matter, you may request an Academic Integrity Proceeding by completing the Academic Integrity Council Appeal form.
  • To recommend a grade of XF, you must send a memo stating the details of the incident and why it merits an XF grade to the AIC chair, and the Office of Academic Affairs with a copy to the department chair, program coordinator, or direct supervisor. A memo must also be sent indicating you wish to assign an F grade to a student who dropped the class before the no-penalty deadline. The Office of Academic Affairs will then notify the Registration and Records office. A grade of XF can only be assigned by the Academic Integrity Council following a hearing.
