Documents Needed to Work on Campus
In order to work on campus, you must fill out the following forms: I-9, W-4 and Direct
Deposit. These forms are available in the department in which you were hired.
Employment Eligibility Verification Form I-9
To complete the I-9 Form you must present proof of identity and proof that you are
legally allowed to work in the United States. (Authority: Public Law 99-603 "Immigration
Reform and Control Act of 1986")
You may choose any documentation from the following:
List A
Acceptable for both Identity & Employment Eligibility
- Unexpired United States passport or passport card.
- Permanent Resident Card or Alien Registration Receipt Card (Form I-551)
- Unexpired foreign passport with a temporary I-551 stamp
- Unexpired Employment Authorization Document that contains a photograph (I-766, I-688,
I-688A, I-688B)
- An unexpired foreign passport with an unexpired Arrival-Departure Record, Form I-94,
bearing the same name as the passport and containing an endorsement of the alien's
nonimmigrant status, if that status authorizes the alien to work for the employer.
List B
Must have one from List B (Identity)
- Driver's license or identification card issued by a state or outlying possession of
the United States provided it contains a photograph or information such as name, date
of birth, sex, height, eye color and address
- Identification card issued by federal, state, or local government agencies or entities
provided it contains a photograph or information such as name, date of birth, sex,
height, eye color and address
- Identification card issued by federal, state, or local government agencies or entities
provided it contains a photograph or information such as name, date of birth, sex,
height, eye color and address
- School identification card with a photograph
- Voter's registration card
- United States military card or draft record
- Military dependent's identification card
- United States Coast Guard Merchant Mariner Card
- Native American tribal documents
- Driver's license issued by a Canadian government authority
(For persons under 18 who are unable to present a document listed above)
- School record or report card
- Clinic, doctor or hospital record
- Day-care or nursery school record
List C
Must have one from List C (Employment Eligibility)
- United States social security card issued by the Social Security Administration (other
than a card stating it is not valid for employment)
- Certified Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350
- Original or certified copy of a birth certificate issued by a state, county, municipal
authority or outlying possession of the United States bearing an official seal
- Native American Tribal documents
- United States Citizen Identification Card (INS I-197)
- Id Card for use of Resident Citizen in the United States (Form I-179)
- Unexpired employment authorization document issued by DHS (other than those listed
under List A)
Direct Deposit Form
This applies to both checking and savings accounts.
In addition to completing the Direct Deposit Form, students must attach a voided check or one of the following:
Acceptable:
- Copy of an account card from a bank, if it includes the 9-digit routing number
- Something from the bank, either a form or the information on the bank letterhead with
the routing and account number (this can be faxed to the student, who brings it in)
Not Acceptable:
- Deposit slip
- Check card
- Bank statement
- Passbook
You cannot complete the information from memory – there must be written documentation.
