Request for Credit Course Fee Waiver Form

  • Credit course fee waiver requests are now automated and can be accessed by employees through the Fee Waiver Benefits Card on the My Missouri State portal.
  • To ensure that the fee waiver has been applied, employees should check the status of the approval of the request online through the fee waiver system. Cash refunds will not be made to the employee.

Employee change of address or name

To update your address or name, full-time and part-time employees (faculty and staff) should use the electronic Change of Address or Name Form - Employee. Once completed, the form will be processed by Human Resources. If you need to change your name or marital status, please attach your new Social Security card/ Marriage license reflecting the legal name/ Marital status change when submitting the electronic form.

Please note that any changes to your W-2 information must still be made directly through Payroll.
